Diabetes care team includes a variety of health professionals to help control your blood sugars.
It all starts with your primary care provider (PCP) which is either a doctor, nurse practitioner, or a physician's assistant.
Your PCP may refer you to other health professionals such as
- Endocrinologist, a doctor who specializes in diabetes management
- Registered Dietitian who provides guidance in meal planning
- Pharmacist provides education on medications
- Registered nurse helps to coordinate your health needs
- Ophthalmologist, an eye specialist who treats eye conditions
- Podiatrist, a foot specialist who treats foot complications
- Dentist provides oral care for healthy teeth and gums
- Mental Health Professionals which are psychiatrists, psychologists, or clinical social workers who will help with the daily and emotional challenges of living with diabetes
- Exercise Professional which are personal trainer, exercise physiologist, or physical therapist and can help guide your exercise plans
Diabetes care and support
Many of the health professionals in your diabetes care team may also be certified as a diabetes care and education specialist. Family members are also an important part of your diabetes management as they may help to prepare meals, accompany you to health appointments, and much more.
